Botox is a highly purified and diluted form of Botulinum toxin type A that works by relaxing the muscles that surround facial creases. As the muscles relax, wrinkles soften and disappear. It is best used for problems on the upper third of the face namely wrinkles in the brow, the '11' lines between the eyebrows, frown lines and the typical crows feet around the eyes.

The effect of Botox usually lasts for four to six months depending on the area being treated.

There may be mild bruising or swelling at the site of injection. Other rare side effects include droopy eyelid, headache, nausea and muscle weakness.

Botulinum toxin is a purified protein. It has been used the past two decades for reducing  facial lines by relaxing muscles of facial expression. The terms botox, dysport, meditoxin, neuronox have become almost household names in some countries amongst some sections of the population.

Botulinum toxin, when placed in a muscle, blocks the contraction or tightening of the muscle. So the muscle is in a state of relaxation. There is no muscle pull on the skin and so lines on the skin reduce, become less deep and may also disappear. It is therefore most useful in dynamic lines, that is lines that occur when one makes a facial expression. In case of static lines (lines that are permanently present), the effect is less spectacular but it can still reduce the depth of the lines by 50% and use of botulinum toxin slows down the wrinkling process.

Appropriate candidates for botulinum toxin include:

  1. People who have deep lines when they use their muscles of facial expression regardless of age. Many people start getting botulinum toxin in the 20’s.
  2. People with lines that are shallow and have only recently become permanent.
  3. People with deep permanent lines may have botulinum toxin, this will reduce the depth of the lines. In case they want further improvement, the leftover lines can be filled with a hyaluronic acid based filler.
  4. People with a squarish jaw may have their jawline reshaped by relaxing a particular muscle just below the ears. This can result in a pear shaped face.
  5. Cleft Chin can be reduced by botulinum toxin.
  6. Excessive sweating of underarms and palms. Botulinum toxin is very effective in stopping the excessive sweating. This benefit lasts a year and the injection needs to be repeated only once a year.
  7. People who want arched eyebrows.
  8. Small amounts of botulinum toxin may be used around acne scars to reduce the depth of the scars.

The injection is done with a tiny insulin syringe and is painless. It can leave a tiny red spot for a few hours. There may be a bruise. Apart from this the side effects are negligible in expert hands. Once injected, the benefit lasts for 6 months. At this time, the lines gradually start to re-appear. The injection should ideally be done 6 monthly for sustained benefit.

Many myths and questions have been raised over safety of botulinum toxin but it has had FDA approval for about 2 decades now and the list of indications with FDA approval is steadily increasing. There have been several animal based studies to prove that botulinum toxin stays in the muscle that is injected and doesn’t spread to the rest of the body.
